This is the model 406M Adam. We still have to find the right schematic and a cabinet photo.
See here the "Common information for the Capehart model pages for the 1930s and 1940s" and for the overview for Series 400 models here.The Capehart model series 400 with Adam cabinet were made in two cabinet versions: The first models were called Adam 402 for 1931/32, followed by 402A (1932/33), 402B (1933/34) and Adam 402C for 1934/35.
Model Adam 406 followed as Adam 406D in 1935/36, 406E, 406F, 406G, 406H, 406K-AM and 406K-FM, 406M and Adam 406N for 1946-1948 with the post war changer 41-E instead of 16-E.
